ALSO PLEASE BRAND THE FORM ACCORDING TO THE COLORS OF THE LETTERHEAD ON THE PDF REPORT

WE OWN A COMPANY THAT ACCOMPANIES PEOPLE ATTENDING MEDICAL EXAMS FOR INSURANCE COMPANIES.
OUR EMPLOYEE WILL HAVE AN IPAD IN HIS HAND AND WILL BE TAKING DOWN VERY REPETITIVE INFORMATION. THEREFORE WE WOULD LIKE A google FORM CREATED WHICH HAS RADIO BUTTONS, CONDITIONAL LOGIC, ETC.

Questions 2 & 3: What exactly do you mean by "contact field?"
20 & 21: Sliders only go up to 10. We can use free text or just set a formula in the linked spreadsheet.
22: There is no question 22 in the spreadsheet provided.
25: In the spreadsheet (column Y, row 4) what does "show identification" refer to? Does this mean that if the answer is "no" then the examinee is to show their ID to the Smart Dog representative?

This can be done, but it will have to be created on our own Google Drives (so you can decide whether the modalities followed are right), and later replicated on yours - either through a creation of a new email account on your domain or through a remote connection, where we can create it on your system. It is double work in a way. Also could you let me know how many documents you would be expecting to be generated in a day? Thanks.
