Contest Brief

We are looking for a new design for our applications that we use when new actors apply to work for the company. We have an old application form that we have used for a very long time, but we want something newer, and more user friendly. I attached the old one to see.

Some Notes on definite changes:

1- checkBox at top for Adults, Kids
2- Headshot field deleted
3- Vehicle, color, plate, state fields DELETED
4- Contact info section, redesigned completely to be simple - 2 phone numbers, 2 Emails
5- New checkbox Field in top section: "Auditions for: Mainstage, Tour, Both"

Black and white only please. Grey is ok, but no color for a printed form.

Do you have your raw logo to use for this one? are you okay with changing up the feel of the form? like say a bit more modern and seelk?
