Contest Brief

We need a one page design. This will be included in a personalized daily health diary. There will be one page for every single day. The user will use it to hand write in their information for each day. We are using this project to get a sense of your design skills as we are hiring a designer for a large project. Don't worry too much about a finished product....just give us enough that we can judge your design skills.

There will be four different sections that need to be included on the one daily page:

1. Food diary. The food diary should have these sections:
- Breakfast
- Lunch
- Supper
- Snacks
It also needs to track calories for the different meals

2. Water. This section needs to track how many glasses they are drinking in a day

3. Vitamins and Medication. This section would be a place to record the medicine you should be taking and the time of day. We want to record the following medications:
- 8:00am - Vitamin D
- 10:00am - Vitamin B
- 1:00pm - Blood pressure Meds
- 1:00pm - Enzyme
- 5:00pm - Calcium
- Before bed - Magnesium

4. Habit tracker. This needs the following sections:
- Stretching - yes or no
- Meditated - how long and at what time of day
- Went for a walk - options should be 3 check boxes or other way to be able to mark it up to three times for the day
- how did you feel today (happy, sad, neutral... use emojis or pictures of some kind to represent this)

We have more work available for the contest winner. Please be creative as we will be judging your design to choose an artist to work with going forward. Going forward, we will be working in Adobe InDesign and Adobe Illustrator, so experience in these programs is preferred.
