Necesito rediseñar el documento adjunto y una version PDF Fillable. Por favor sea creativo y utilice el logo de mi compañía también adjunto.

Importante:
1. Que se vea profesional/corporativo
2. Que incluya todos los fields y toda la información según esta en el documento
3. Debe quedar en no mas de 2 páginas de largo
4. Cuando me entregue el arte incluya el formato Word
5. MAS IMPORTANTE QUE TODO: También incluya una copia de ese documento en un formato PDF Fillable (ver ejemplo adjunto donde la persona no puede cambiar el documento pero si puede escribir en las areas necesarias)

I need to redesign the attached document and a Fillable PDF version. Please be creative and use the logo of my company also attached.

Important:
1. I need it to look professional/corporate

4. Delivery should include Word Format
5. MORE IMPORTANT THAN EVERYTHING: Also include a copy of that document in a PDF Fillable format (see attached example where the person can not change the document but can write in the required areas)
