I have 20 PDF's that need to be converted into fillable PDF's.

In each PDF there is a black box that needs to have the ability to attach a photo from a folder on the person's computer.

The fillable part of the PDF isn't too difficult but the PDF must have the ability to add an image and save the PDF with image.

Attached is a sample PDF to look at. Some will be shorter than the one attached.
