GDPR compliant Privacy Policy template / Website Terms of Use

I am looking for somone to provide or help create a privacy policy (and or) website terms of use documents. Not to just pull up some template from online (because believe me, I have looked) but to help me tailor something that is accurate to my needs/requirements and ticks the boxes so to speak.
With specific regard to privacy policies, the following are on my list of items to ensure included;
- Collection of form data entered by users when submitting an online enquiry (name, business name, tel, email, mobile no., address, nature of enquiry)
- Collection of form data entered by users when submitting an Opt-in request for marketing emails or mailing list signups (name, email address)
- Cookies and any other associated files for Google Analytics to function
- Social Media plugins / embedding of feeds; twitter, facebook like buttons, fan boxes, feed activity boxes, youtube videos.
Sidenote: With regard to the first 2 points in that list, the data 'collected' will be stored digitally on internal computer systems and archived.
The policy should also cover any other aspects required by law.
Website terms of use should essentially stipulate that the user agrees to the collection of data / transmission of cookies (as described in the list above) if they continue to use the website and accept the associated terms of the plugin authors (facebook, twitter, etc) and that they abide by and agree with out privacy policy (link). + anything else you feel necessary to mention.
Happy to discuss this further but I am looking for someone with some experience and knowledge in the area who maybe has completed or compiled a few policies for their websites or clients websites, to assist with this project.

Website:
3-4 pages. Simple html/css, brochure style website; homepage, our services, portfolio, about us, contact us.
Site contains 2 web forms; enquiry submission and opt-in to email marketing (exact fields mentioned in primary project description).
The privacy policy needs to cover the points mentioned above + any other relevent aspect of GDPR. The website informs users of who we are, what we do /offer (our services), shows a portfolio of previous projects and provides contact information.
To use the website, users must be aware (via the privacy policy) of our use of google analytics. Possibly social media plugins, such as; twitter timeline feed integration, facebook like buttons, feed activity and fan boxes and instagram integrated photo feed and all files/cookies associated with those plugins. The collection of the users personal data IF they submit it to us via one of the web forms and all of the bullet points above.
Looking for someone with GDPR knowledge and privacy policy writing experience to write a privacy policy that covers these aspects and is GDPR compliant based on the website information provided above.
