Skills assessment survey

I need a skills assessment survey that would work with both paid and unpaid (volunteer) staff to see what skills we have available to us to focus on customer service, marketing, management, program development, people management, computer skills, computer graphics development, public relations skills, graphic design, other certifications and education, etc.. We have multiple teams and departments: Finance, Administration, Nutrition, Fitness, event planning, Activity planning and implementation, Home care, Case management, Volunteer Management, Human Resource Management, IT, Community Development, Grant Writing. We like to promote within but are not sure what skills we may have on staff that might be a good fit for other departments / teams. This assessment can help us find that out. We also have multiple projects we want to implement that need skills such as access database management and building, partnership building and need to know what skills we need to look outside the organization for that we do not have on staff.
