Advanced Web Forms Needed

I need a website form built and one cleaned up on my website. The one I need built is an extensive application. I have the application in PDF form, and need to have it converted to a form on my website. When submitted, I need the form to either be emailed to me in PDF form, or I need to be able to access it on the back end and be able to download and print it. The second form is already on my site with the using Ninja Forms. I just need it cleaned up.
So please have a lot of experience in advanced website forms, and also provide examples. Also preference will be given to those who can start on this right away.
