Based on Ozeki VOIP-SDK to build a softphone. To enable Audio layer on Rpi, and use USB mic. No desktop GUI, only a service daemon, and simple web interface or API to make own on top. Audio full duplex and video support as well. As an option can consider an open source Linphone (or vaxvoip), if will run smooth and stable.

Ozeki is pretty nice for softphones. I can give the open source alternatives a look too. A little more information about your setup would be helpful.
