This sounds like a very straightforward project, and I have done several similar databases in the past. I can turn this around for you an a day or two if you are just after the database and forms (no data entry). I would first examine your paper forms to determine the most effective structure of tables, which I would run by you for your approval before building the rest of the app. Hope to hear back from you.
