I have a wordpress page.
I have a protected page with a single password.
I want a form that will allow a user to submit info such as:
Name: first, last
Location: 1 / 2 / 3 / 4
Department: department
Budget Code: abc123
Waiver Request: yes / no
Dates: from and to or on
Permit number: 1-200
Upon submission an email will be sent to who I designate conditionally for each location.
The person who gets the email can hit approve or deny and that will automatically send an email to the form submitter with a pdf file attached. If I could attach the dates requested onto the pdf that would be preferable.
If denied the submitter receives an email with a preset response and requests they submit the request again with a budget code.
If they submit with a budget code the request is queued and a user can login to the form submissions to review the request. That user can then hit approve and an email will be sent with a pdf file attached with the dates requested preferably.
I would like the fields for the completed requests to be exportable to an excel spreadsheet.
I will upload a complete form template along with conditional logic tree.

I have a wordpress site. I have staff that need to submit parking request forms online. I want to protect the form so that staff can access it while the general public can not. I want to secure the page with a password either through wordpress or built in to the form.
The form will request the following:
Name:
Email:
Campus: (drop down 4 options) ARC, CRC, FLC, SCC
Sponsoring Department:
Event Title or Visitor Name:
Event / Visit Description:
Date Requested: Date of or dates through.
Event Location:
Estimated Number of Visitors:
Number of Permits Requested:
Waiver Request: YES / NO
If Yes: Follow Waiver Request Logic Map
If No: Budget Code:
I would like to have an admin area where each request can be viewed. If a waiver is requested an email with the above information will be sent to a unique email according to the Campus dropdown. I would like the person who receives the email be able to click a link to approve or deny the request.
If approved I would like it to be marked complete in the admin panel queue. I would like an email to be sent with a PDF with fields generated with the unique id that corresponds to each request, dates to and from, and a permit number. I have included an example PDF.
I have also included an example decision tree.
If they select no for waiver request then the option to enter a budget code is made available. A budget code must be entered to complete the request. The submission is placed in queue. Admin accesses the panel and reviews the information. If marked approved by admin an email is sent as explained in the above portion. If denied they will receive an email with a preset denial response.
