Forms fields save to excel and 2 folders *BASIC BASIC*

This project is extremely basic I just do not know how to use dreamweaver or basic html forms. I have a submission form that has 13 fields and a submission button. I need the fields to save to an excel sheet that is in the same folder of the html page. Each time a form is fully submitted a counter increases to keep track of th submission, the field empties and the next submission populates the next row. Similarly I need two upload fields to save to two folders in the root folder. One of the fields is Dependant on another field. One of the upload filenames needs to have its named changed upon submission to match the first field. That is the scope of the assignment. It is extremely basic I am pretty sure anyone here can do it in less than an hour. Please if you bid be able to do it in the next 12 hours, and also be available for modifications.
