Basic Data Gathering and Output Framwork

We need a developer with HTML, PHP, and Mysql abilities to build the framework for a site that we're going to use to capture our customer's success stories. There will be three parts (See basic layout comp attached). ## Deliverables We will basically need a 3 page framework that follows this plan: Page 1: This page will start with a drop down menu where the user selects one of our 9 products. Next will be a series of forms where they would fill in their Full Name, City State, email, then a large box where they type out their success story. Below that would a button where they can attach their photo and another button where they could upload an audio, video, or document and attached it with their story. Below that would be a 1-800 number where the user could record their success story (would need to program this mp3 recording to auto-attach to their database file.) and it would all end with a "submit" button at the very bottom. Page 2: After submitting their success story, the customer's data would have to be stored in a database and an email would need to be triggered to be sent to an admin's email address. On this 2nd "thank you" page, we would need to have 5 photos and accompanying descriptions of 5 free gifts that they could send to their friends and family by referring them. Below the 5 gift descriptions we would need to give them a series of forms numbered 1-5 giving them a space to enter 5 names, 5 email addresses, and then 5 drop down boxes that allows them to select which of the free gifts they want to send them. Then below that would be a space where they could enter their name and email address. And below that would be a pre-populated subject line and message, but still editable. Lastly a submit button where the emails would be sent to their friends and family from their email address. At this point we would still need some type of script that captures the email addresses for us, but again, still sends the emails out to the referrals via the user's own email address. Page 3: This would be a simple "thank you" again page that just has 5 links to the free gifts allowing them to download any of the gifts for themselves if they like. Other needs: We would need to have code that could display these Success Stories on our home page in several different sized boxes (300x250 or 728x90 for example) and we would need the following options built in so we could sort and customize what's displayed by these parameters: *Products (1 or many) * Pic or no Pic * View * Random * Date range * Most recent / oldest * # to display * Auto refresh w/ most recent submission Database needs: We would need to take these collected stories and information and be able to: * Configure: email to send new test to * C.R.U.D.: All records * Would need the ability to run reports based on this information.
