Add Calendar and Mobile Phone prompt to online form and sync with email database

I run events on a wordpress website. When an event is listed as sold out there is a JOIN WAIT LIST option that activates. When customers fill out this form, they are asked for their Date of Birth and Mobile number. For Mobile, I want the box to prompt: 04__ ___ ___ so the customer must put in 8 other digits to make 10 total. If they are accessing my website via their mobile phone, I want their keypad to display. They are also asked for their Date of Birth. I want the calendar to pop up to allow them to input their date of birth. Currently, my site won't allow for this. I am getting error and won't allow form to function - if same form is prompted in one div/container. This info then needs to be stored in my moosend email database and send me an email notification that someone has subscribed. See upload for details
