Build an online database

Dear Freelancers,
We need to create an online, straightforward, database.
Here are the prerequisites:
1) Login or Sign Up page
2) Home page with two tabs in the menu:
a) Activity
A table with 10 different columns for each client request on each line
b) Client Request
A form to fill-up with a [SUBMIT] button that posts it on the table of the Activity page.
It's very straightforward.
In addition, we need three levels of access:
1) Administrator (us)
2) Director Access (can see the Activity page with the tables of all existing accounts)
3) Regular Access (can see its own Activity page and the Client Request form)
=> The project may evolve in the future with new features to implement.
