Build a very Small Combox box based Application

Need to populate a combo Box using PDO from a database . Once an item is selected , fields are displayed extracted from a table. On clicking save , the fields displayed to be saved in another table. Message Box displays the item selected. Better is stored in classes and functions created. Will give more information once you have been selected. Budget only 5 to 10 dollars.
