Develop website for small village (1100 households) to identify and report on local issues

Using United States GIS land data, build a website that shows the individual properties in a small Village (approx 1100 households). The website will allow a household to register a score on a number of issues and then allow the ability for reporting and a "heat map" of the issues and scores. I can provide an example site that has similar mapping capabilities if needed. This project requires the following: 1. Duplicate the map for the small Franklin Village section of the map (see attached picture). 2. When a user clicks on a property, display a popup window to that allows the user to answer 10 questions with a numerical score (1-10). Store the lot address, owner and scores for the (10) questions for retrieval later. 3. On the left side of the map page, Add a search function allowing choice of questions and score values. i.e. Show the properties that have question #1 scored as "1". 4. Add a "results" page to the website that will show the (10) questions and the score results as a histogram graph.
