Need to build and deploy a simple webform to collect customer feedback. Collecting contact information, with some dropboxes and radio buttons to describe some details more clearly Once completed the contents of the form should be emailed to a specified email address. No database is required.
Form needs to be hosted on IIS - the rest of the site is built using asp - the current feedback form is built using PHP.
Current form is spread across 5 forms - samples from 2 that will be consolidated shown, complete brief coming from client presently and will be provided before project commencement.
Will require assistance with deployment, be that providing the files in a deployable package or working with web team to ensure page works to design upon implementation.
Need within 4 weeks.
