I need Terms and Conditions confirmation modal on form submit

I have some minor tasks on my WordPress site to start with. More elaborate tasks to follow successful completion of minor tasks first. I am looking for someone who can be committed.
I have a post item multipage form. On the last page, after user hits submit, I want a terms and conditions modal. there are already many modals on site, so it's not a new modal you don’t need new jQuery or CSS. The modal will, as usual, have modal header, modal body and modal footer. Modal header will have title/header; modal body will have text (terms and conditions text); and the modal footer will have 2 buttons, CANCEL and CONTINUE. CONTINUE will simply submit form, you don't have to do anything else. CANCEL will close modal.

All text must be replaceable in admin, as other text is already. Terms and conditions is a lot of text, maybe 3-4 standard A4 pages. So I need fixed modal height with scroll. I want to write this text into a standard WordPress post or page and then you extract it from post to modal body, maybe by short code. And finally, this modal will have to be reusable, so you can place it in another form. All up, $40. Then there’s more tasks in the following

• generate csv files out of sales data
