I need a website built

once built i will maintain, I work in cell phone and accessories, I sell services along the lines of New Devices, Accessories, I remotely Root android devices, Device Repair and i also offer device insurance as well. I have started the website but theres somethings i would like to add before i dig in any further, just like once a customer selects an insurance plan, i need a form to pop up next to get all their information etc. right now it goes straight to checkout, I also would like them to be able to manage their coverage on the my accounts page as well as make payments. and make a claim if needed. the web address is www.okbinsure.com.
