PHP/Ajax MyLife/Spokeo like functions

Am in search of a developer that can create a site similar to spokeo or MyLife, only I need it to query a database on my system. When a user visits my site they will be provided options for search criteria. Example:
Nickname [Form input]
DOB [Form input]
ZIP Code [Form input]
Mobile [Form input]
E-Mail Addr [Form input]
If any one of those variables returns positive a notice should posted not NO FULL DATA would be shown. For example here is what my table would look like:
Nickname DOB ZIP Mob EMAIL
JCash 1980 /11/05 90210 2145551414 n@[login to view URL]
If someone searches JCash and n@[login to view URL] and there are two matches, then the page should show something like: FOUND - for more information click here.
The “found page” should contain a bit more information redacted. For example, (remember the user searched JCash and n@[login to view URL]) the page would show:
Nickname: JCash
DOB: 1980/xx/xx
ZIP: 90xxx
Mob: 2145xxxxxx
email: n@[login to view URL]
On the “click here for more” link, prior to data being disclosed, this portion is a paid function, and should tie into Paypal. “To unlock this information, please register for a paid account” I would need the person that paid for the account to be able to make no more than 5 queries in a 30 day period. This can be done with a cookie/supercookie, etc.
MUST make sure there is input validation, DO NOT want any SQL types of injection.
