Prototype Website- upload download docs using blockchain technology

2-it should have an upload button to upload documents
3-it should provide time and name of the person uploading the document along with the hash id of the transaction.

4-the document needs to be saved in any database
5-there should be an option to select any uploaded document from the list and download it
For the deliverable I will need the following:
1. Source code before and after the implementation of blockchain.
2. Working prototype of online document submission implenting blockchain
3. The prototype should have retrieve functionality where user can retrieve a document if required
