Scrape site and create new lead gen funnel

I want you to take this site, scape it, I don't have the files. [login to view URL] Put it up on [login to view URL] Then after the question in the quiz, put an aweber or mailchimp optin form where it says Show Me My Score, I want to make that Send Me My Score And add an aweber or mailchimp optin form below it. After optin, then goes to a new page that looks like the same Show me my score page, that is the optin confrmation and the button now says Learn More Now And that goes to a link I give you, probably for testing a youtube video. Here is the info on the scores. I really would like to pass this to Aweber/Mailchimp as they optin, the form will just be Name and Email, then pass this score with the optin as they submit so I can use that in the follow up emails. This part doesnt have to be done if its too difficult but I would prefer it setup this way.
