work order program. -- 2

looking to create a work order program similar to the one in the attachment and i want it added to my site. it will the backend for vendor. the upload shows what a type work order will look like. There will also other work order that not as long as the one shown in the pictures for example, there will be work oders for grass cut and snow removal etc. The one i have uploaded is the longest type of work order. These work order will be issued to vendors as assignments. admin will be able to issue delete and assign new work order to vendors. This program will be added to existing site. I have upload screenshot similar to what the end program will look like. ending in END.PNG. The work order form will have validation for each question
