Flight Simulator

I operate a flight school and have a flight simulator used for training students by instructors. What I need is... when a student launches the software for the simulator, the first page to be a Disclosure and Consent page that states they cannot log the "flight hours" in their flight log book (i.e.: it does not count as actual real flight time) which I have already drafted. I just need the Disclosure & Consent page to open up when they launch the software and have them type in their name on the bottom as their "consent" before the software launches. The last thing I need is a timer to start counting the time the flight sim is used once they launch the software until the close the software so I can bill them based on the time shown on the timer.
