Build a mobile responsive shopify homepage with some custom functionality.

I am looking for someone capable of designing a Shopify homepage. I will be selling 1 product, in different quantities and across two separate platforms.
On the homepage, I would like two options for the customer to be able to select their platform (Xbox One or PS4).
After the user clicks that, I would like the page to load a quantity selection, with a slider, buttons and text input box to allow the user to select the quantity.
I would then like to give the user a text entry field, where they can start typing a name, and it will supply a drop down box with suggestions, that they need to click. I can supply the database needed for this.
I then need a "starting price" field that cannot be edited, give the user a number and displays the quantity they have selected.
I then need a checkbox that says the user confirms their actions, and then a buy now button.
This should then add the platform, product and quantity to the cart, and take them to the checkout page. The details entered on this page will need to be stored with the order in the admin panel.
