I would like to have a responsive app/form that can be used offline in a web page. It is a tool for Paramedics to use in the field to triage potential stroke patients. I have attached a screen shot of what I am looking for in terms of GUI. The app/form would work as such. You make your selection for facial droop, arm strength and grip test and the score is added tallied in the lower box with recommendations about how to handle it. I do not need the copy results or next step button in the example.
