Build a Survey with Assessment Results - 03/10/2018 06:16 EDT

Hi guys, My team is looking to do a survey in a rating scale format, and is also looking for assessment results to be immediately churned out for the respondents’ viewing upon completing the survey’s questions. We were wondering if anyone is keen to take on this project? We have the survey questions ready and are looking to get this ready by 19 October. To illustrate, we will want to assign scores (etc. 0-3) based on the participant’s responses (eg. from disagree to strongly agree), and at the end of the survey, tabulate the scores to derive either a high dimension, medium dimension, or low dimension and include some description with the results. (eg. scores from 0-5 will belong to a low dimension, 6-9 will belong to a medium dimension…). It will be good if you could refer to my attachment to have a clearer understanding!
