Simple Windows Application to print Receipts

I need a simple application developed so i can print receipts for customers.
It needs to have the following
- Input member detail screen
- Input receipt detail screen
- print receipt in A4.
- All receipts saved in database and searchable using criteria eg receipt number
Please see attached sample receipt, i would expect it to look better than this but this is the info that is required on it.
I know its a simple project so please bid accordingly as my budget is limited. If your bid is too high i will have to disregard it.
