Small Web Application in C# 4.0 + MS SQL Server

I need a small web based application in C#. I have 5 templates in PDF format you need to convert them in to appropriate format (html etc.) or suggest best way to store them. Application have login page for admin only not for any user after that I can select a template type which I want to print and below that I can fill all the information through text box, drop down, check box and also upload images. After submit I will see are preview of the filled template and finally I can print the template in colorful. Data should be saved in MS Sql server data base and can be retrieve any time if needed. Plz come up with best price and time frame also. Plz Read complete requirement first and then bid, your bid will start with R letter on the top, and only those bids are accepted.
