Simple File Upload form (.aspx)

Hi,
I require a simple webpage form that will allow a user to select a file from their local system, and upload it to my server. During the upload process, i need the filename changed on the fly and saved as "[login to view URL]" very specifically. I do not want to have the user worry about the filename. I also want it to say either "successful" or "failed" in some way so the user knows what happened.
The upload would be to a folder like: c:\inetpub\wwwroot\demo\images\
The [login to view URL] will already exist each time, so it must be overwritten by the script upon upload.
My IIS server is [login to view URL]
It is running on Windows Server 2012 R2
The page can be very raw, just working. I can implement it into my own site.
Should be simple and not take long at all.
