Customer Vehicle handover form

I need a printable A4 form designed, that can be given to printing companies to print a pad, as well as printing myself. The form should have a graphic of a car where we can indicate any existing damage. I have attached a sample form design that I created on Vista Print - this however is for a much smaller form, I would like it to be A4, and then scaleable down too. This has no graphic either. I have also attached another form for reference. I would like the final files to be in PDF format, as well as formats I can edit - photoshop, illustrator, and indesign. Alternatively, a design that can be modified in Microsoft word would be excellent - perhaps you can design the headers and footers graphically, then insert into word. I have attached my company logo and a sample form, showing the colours. Modern clean fonts such as futura and helvetica neue. Purple colour is #643389 that I use in my business cards and website. Form should look clean, crisp, modern. Text should be able to be edited by me easily at a later date - prefer word format for this reason, but photoshop is ok too. The legal terms where they sign should be as follows: Payment The client understands that all prices are indicative to the works being carried out and may reasonably be varied if required. Obsession Car Detailing Pty Ltd are under no obligation to commence work without a deposit being paid. In addition, Obsession Car Detailing Pty Ltd may request further payments after reaching practical completion of each stage. Indemnity The client understands that Obsession Car Detailing Pty Ltd discharges all liabilities that may arise from undertaking the works performed on the motor vehicle. The client understands any employee of Obsession Car Detailing Pty Ltd will need to operate the motor vehicle insofar required to complete the works. The client discharges Obsession Car Detailing Pty Ltd for any loss, damage, theft or fire in relation to the motor vehicle and the items inside the motor vehicle. Promotional and Advertising The client understands and agrees the motor vehicle may be used for commercial purposes and may be shared on social media, internet, and brochures. The client agrees to allow Obsession Car Detailing Pty Ltd to use the motor vehicle and promote the business insofar as they wish. Timeframes Obsession Car Detailing Pty Ltd endeavours to complete the agreed works in reasonable time frames. However, the client understands for reasons beyond Obsession Car Detailing Pty Ltd that sometimes the time frames may not be met and will be notified accordingly of any extension of times in order to complete the works.
