Script based on blueimp jquery uploader ( Need Fixing - URGENTLY!)

This is system for uploading portfolios for students. We already have about 6000 file entries in the database. The script is mainly for uploading single or multiple files. The single files working fine (when you upload one file) But when you upload and select different values on a dropdown form all the other files gets the same value from the dropdown if you upload them one by one it will work but when you upload more it will not work i mean all the files will be with same dropdown file. The same is when you editing the files if you edit one file it will affect all the files. [login to view URL] For each user we need all the files that he uploaded to be shown when the user is logged in. I dont know why but with the same code it was working properly. We only changed from radio-buttons to dropdown (title selection) and now the script dont load the uploaded files. You don't have to make new query for it or something like that. Just check why it's not loaded now, it was working fine before. All the files that were uploaded was shown for every user. I need only experts to apply to this job. If everything is finished ontime and properly i will give some bonus.
