95343 Simple Math Tool

We would like to create a web based math tool that calculates the following: For question number 1, the “Amount” field should be filled in by user. We would like the time periods to appear as a drop down menu – month = 31 days, quarter = 12 months divided by 3 months, 1 year = 12 months. For question number 2, the “Amount” field should be filled in by the user. The “period” field should be drop down. For question number 3, the GSB rate should be automatically populated by dividing the “Amount” in question number 1 by the “Amount” in question number 2. The result of question 3 should always be expressed as a percentage. The size of the window for the tool should be easily adjustable. We want the tool to appear on our home page but it should not take up the entire page. We also want the ability to send the tool as a link in separate emails.
