Simple Wordpress Signup Form Customisation

** Please see a better formatted version of this post attached ** Configure Signupforms / Custom Post Field The theme that we are using has a custom signup form for Listings and Events and we would like some custom work done to the signup forms. It would be ideal if these forms could be AJAX-ified, though not necessary. Bidders would need to confirm this BEFORE the project is awarded. The theme does have some built-in features regarding Custom Post Field’s, but these are beyond my limited ability. I can provide the supplied theme setup documentation if necessary. 1. Address. a. Instead of a simple address such as Unit 7, 234 Vive Street, Hocking , Western Australia, 7832, we would like the address form to be broken down into : Custom Post Field Example DwellingType Unit DwellingNumber 7 StreetNumber 234 Street Name Vive StreetType Street PCode 7832 Locality Hocking State Western Australia *Postcode – when visitor inserts their PostCode, we would like for the Suburb and State to autofill. We have a .csv file that has the appropriate data and will provide when necessary. 2. Time a. This should be changed from Time to “Service Time” b. We would like the ability for the visitor to add multiple times, not just one. This could be with a series of drop down boxes ie Day, Time, AM/PM. Do you have any suggestions? 3. Phone a. Add a two digit area code box with brackets “()” on either side b. The Phone Number should be reformatted to “(xx) xxxx-xxxx” 4. Category a. This should be a series of horizontal drop down selection boxes that list Parent Category and Child Category. For example Parent Parent Parent Parent -Child -Child -Child -Child -Child -Child -Child -Child -Child -Child -Child -Child * Visitors should be able to select at least one Child Category from each Parent Category. Note: 1. These changes need to be in effect across both signup forms for Listings and Events. 2. These changes would obviously need to be referenced and styled on the actual Listing / Event details post on the live site AND the Dashboard which forms part of this project 3. All of these changes will need to be commented and logged and must be compliant with best practices. 4.
