Straightforward PHP/MYSQL project - set up MySQL tables & and PHP forms

I need some PHP code and MYSQL tables created. I need this fast. Four tasks: 1. Create PHP code to generate 6 tables you'll find in the attached file. If you know a better way to format the table columns, then go for it (and let me know). 2. Simple security + forms * Password security for files in "admin" folder you'll see in the directory (with login screen asking for password from "admin" table) * Create forms that allow me to add, update and delete records to each of the 6 tables 3. Create these 5 unique pages 1. Output one record from the artist table that corresponds to a URL variable, "art_id" passed to the page. If the URL variable is not present it should give message "Sorry, artist not found" 2. List all artists in the "Artist" table if "art_active" is "yes". 3. Output all Artists, and each artist who belongs to one or more bands from the Bands table should have those band names display next to them, and each band name should be a link that contains the band_id primary key as a variable.(I may not have set up the sec. keys correctly. If I didn't then please fix this and let me know what you did). 4. Output all bands and next to each one show the artists that belong to that band. Artists names are links that have the "art_id" in them. Some bands may not have artists. 5. Output all genres and lists each artist that belongs in that genre next to it. Artists may belong to multiple genres. 4. Create a form w/one check-box. When submitted, appends the vid_id (which will be present) to the value of a cookie. This is to allow people to add videos as a "favorite" * Cookie should already exist, but if not the code should create it. * Create code that recognizes the vid_id values in the cookie and then outputs list of "videos" and matching "artists" ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
