Custom PHP Landing Page & Script

This job is to create lead generation page with tracking capabilities. It would work like this: 1) Landing Page This is where the visitor to the page would enter information such as their name, email address ,telephone number etc 2) Submission When the form is submitted it should store the lead in a user area & admin area. The admin assigns what users have access to the information that was submitted through this particular form. The user area would show all the details that were submitted. It would also email that lead data to the email address of those users who have been assigned access and also the admin. Basic options such as pagination so that all leads don't show on the same page, for example 10 lead listings per page should be built into it for formatting purposes. Be sure to also have the lead show the date/time that it was submitted and allow leads to be sorted by this. 3) Admin area This is where the admin can login and add/edit/delete users who will be given privileges to see the leads that are being submitted. Admin would also be able to view/edite/delete leads that are in the system. That's it. A simple script.
