Add drop down menu to 3 forms in Cold Fusion and link to MSSQL Database

This is a simple and easy task. I need a drop down menu created and to be linked to a MSSQL database. The drop down menu will be of 6 items long. The site is coded in Cold Fusion. Database is in MSSQL 2005. There are already examples of drop down menu on site, we just need one more linked the same at the others. The site has user's logins, the user enters site through a secure page. Then the user fills out 1 of the 3 forms and submits them to the database. The forms are printable and searchable in the database. **This has all been created. We just need one more drop down menu. The Coder must fit the drop down menu in the? appropriate? style of the form to keep a? consistent? design.? The drop down menu must have a validator attached to make sure the user chooses from the Drop down menu. This needs to be completed by 10pm my local time. -Tim
