AJAX Order form For my Product Order Page

Hello, I would like to have a order form with some simple requirements. I have a product with 3 different packages. When the person clicks buy now on my order page then it get to another page where they see a order form and place their order with their shipping, billing and credit card info. I want Step one at the top giving the information like how much is the package price and how plus shipping the total and the bonuses they get. And then the is under step one step 2 it lets them put their shipping address and then on the left there is billing address but if they check same as shipping it auto fills and shades out. - I want it in Ajax with no page reload when they place their order the thank you message and summery of they order comes in the same page with no reloads and I get all the order inputs in my e-mail and in a Database which I will provide. - The form must not be with simple input the inputs must be styled with CSS or something.
