Simple web application

NOTE: PLEASE READ ALL OF THIS BEFORE BIDDING Overview: We require the programming of a simple web app for our own use. We currently: 1) Visit a pre-defined website 2) See if the site has a feedback or contact form on one of its pages 3) note in our information if it does or doesn't We need an application that speeds up this process as we are using an Excel file at the moment. Here is how I see this application working: 1) Team member visits URL and clicks a button "start session". Records the start time 2) Clicking the above button then displays a web address on the page - pulled from a mysql database. Clicking on this URL opens up this web address in a new window. 3) Once the team member has completed what they are doing, they close window and come back to this page. They either click "Yes, has form" or "does not have form". This information is then saved as "yes" or "no" with the web address in the database along with a date/timestamp of when it was [login to view URL] would have something like: [login to view URL] yes timestamp [login to view URL] yes timestamp [login to view URL] no timestamp 4) web app refreshes page with the next URL/web address from the database and process starts again 5) We need another page to download all the results as a CSV file. Once results downloaded they will be removed from the database if a checkbox on the page is clicked - ie "remove results after download". The columns of the CSV need to be web address, result, timestamp We will provide the design. You are just required to create the programming for this simple application NOTE: - We are interested in long-term relationships as we are looking to expand our team of freelance PHP programmers. - please provide either sample files or point us to sites you have programmed. A bit of background information about yourself would be useful also - we like to know how we are working with I look forward to working with you ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Linux, mysql, PHP
