Create MYSQL Database to communicate w/ PHP Website

This work request is "for hire" only. Our company will retain all copyrights, intellectual property, trademarks and license of the database created. The project involves creating a database in MySQL that communicates with our website that was written in PHP using Drupal. The database to be created will receive information entered into our website by clients and also have additional fields on the backend. Other features include the ability to post comments we enter into the database and make the information viewable by clients who log into our website. The database must be built to be expandable for pre-planned future projects as well. A few key features of this project include the ability to have different log in "authority levels". A master log in will have full access to make changes on the database. However, other log in authority levels will be "limited" or "view only" access. The database must also be able to "Form fill" 3 different documents already completed. The information to form fill will be directly from fields of the database to be created. The database also needs to have easy to use query tool to search accounts by address, name or case status. We have already determined most of the fields needed and also have a high level overview of the pages of this database on paper as well.
