VERY URGENT: fix add media issue on wordpress -- 2

I have updated to the latest Wordpress version. They came up some issues on the posting. 1) The Visual Bar in the add new post comes differently than it is supposed to be shown. You have to fix that How it is now: [login to view URL] How it is on another WP i have: [login to view URL] 2) When pressing the "Add Media" it comes up messy. Fix that, as it is supposed to be. 3) Check if everything else is working fine.
