Auth Net Order Form

I am looking to have a single page "payment form" that can process a payment via Authorize.net. It will need to process auth/decline codes displayed (approved, declined,address mismatch street/zip, cvv mismatch). I would like the form to include: Invoice # First Name Last Name Billing Address1 Billing Address2 City State Zip Phone Number Email Credit Card Type (MC/Visa/Discover) Credit Card # Credit Card Expiration Credit Card CVV Code Submit/Reset buttons Here is what I am looking for: [login to view URL] Do NOT send examples of web design...
