Build a questionnaire form

PLEASE dont bid more than 55usd as i wont look at your bid This is a very easy project if you are a ninja form developer =) Please take your time reading the specs. i want a bid that isnt copy paste, i want a bid that is related to my project. The project is a list of questionnaires. The admin has to be able to change all text in the forms, including the questions. Like in contact form 7. There is 40 to 50 questions in this project. The project is a list combination of questionnaires -- i myself am a coder so i can safely tell you that the 3 hard points i found in this was: 1 - Add a product of user selection to cart 2- when a user selects for example 'no' he goes x steps back (and if he again selects no he goes x steps back - kinda like a loop) 3-The questionnaire should also be able to printout a list of products that they chose, for the client and for myself.
