DropzoneJS Uploader with formdata

Looking for a developer to write a simple uploader script that uses dropzoneJS as the frontend. The script needs to be able to follow the workflow below: User sees the option to drag and drop or click to upload . Once the user has added the uploaded file or files a div appears with the option to add a description of the upload and then the user clicks submit to upload the files. The uploaded file is sent to a [login to view URL] file where the script should also handle the storing of the file and the ability to save the description text into a database along with the file name. Would prefer pdo or laravel backend. Then have a page displayed saying upload complete.
