I am looking for a coder that can re-design the layout of a [login to view URL] document to a more user-friendly look and feel and convert it to an editable PDF document. In other words, a user must be able to open and fill the fields in PDF. After filling all fields I want an email button in the PDF which will allow the user to email the document to a specific email address. This project is urgent. Just to be clear, the project is two parts: * Redesign layout to more professional look and feel * Convert to editable PDF document
