URGENT!! Quick addition to PHP/mySQL customer tracker

Here’s the situation. I NEED THIS DONE TODAY!! Not tomorrow. I WILL pay today. I work for a credit repair company. I hired someone from here about 2 weeks ago to build me a system that we could use to track the progress of our customers credit repair process. It works great! Basically, we pull someone’s credit report, enter in each item on the credit report into an admin interface, and then the customer can view that information we post from a user interface. There are different URL’s for accessing the user side and admin sides of our system. When we login to our admin side of the tracker system, we can select which customer we would like to work with by entering in their customer ID that gets set by our order system, independent of this system. We then have three buttons at the top, one for each credit bureau. To add an item, we click a button at the top for which credit bureau we would like to add an item to. We then click add item, a screen comes up for us to enter information about that item, we click submit, the item gets added to whatever credit bureau we were looking at when we clicked add item. The problem with this is that if there is an item that shows up on all three credit bureaus, we have to enter that item three different times in order to make it show up under each different credit bureau. I would like to make it so when you click add item, after you enter whatever details you want to enter, you have three check boxes, one for each credit bureau, and check each one you want to apply that item to. So you would be able to type in an item and choose to list it under 1, 2 or all 3 credit bureaus. When you’re looking at the main admin screen for a user ([login to view URL]), you see all of that user’s items and have the ability to modify, add, delete, add notes to items, etc. When you click “Add Item”, it brings you to the add item dialog ([login to view URL]) This should be the only script you need to modify, should take 10 minutes for someone who knows what their doing with php and mySQL.
